Before you begin...

YOUR APPLICATION CHECKLIST:

  1. Submit an official transcript from every college/university attended since your last enrollment at Florida State University.
  2. Complete the Florida Residency Affidavit. This is required for all readmission applicants who wish to claim Florida residency for tuition purposes. Students who were previously coded as non-Florida must go through a reclassification process through the Residency Section of the Office of the Registrar.

UNDERGRADUATE STUDENTS: - Complete this application if:

  1. You were previously enrolled in an undergraduate program at Florida State University and have been absent for two or more semesters, including the summer.
  2. You were academically dismissed from Florida State and have been absent for two or more semesters, including summer. (Academically dismissed students are not eligible for readmission unless they have been reinstated by their academic dean. Reinstatement to continue does not guarantee a favorable readmission decision or admission into a specific major.)
  3. You withdrew from Florida State and have been absent for two or more semesters, including summer.
  4. Your last term of enrollment at Florida State was administratively cancelled and you have been absent for two or more semesters, including summer.
  5. You earned a bachelor's degree from Florida State and wish to pursue a second bachelor's degree.

Note: Readmission will require approval from your academic dean with final approval by the Office of Admissions.

GRADUATE STUDENTS: - Complete this application if:

  1. You were previously enrolled in a graduate program at Florida State University and have been absent for two or more semesters, including the summer.
  2. You were academically dismissed from Florida State and have been absent for two or more semesters, including summer. (Academically dismissed students are not eligible for readmission unless they have been reinstated by their academic dean. Reinstatement to continue does not guarantee a favorable readmission decision or admission into a specific major.)
  3. You withdrew from Florida State and have been absent for two or more semesters, including summer.
  4. Your last term of enrollment at Florida State was administratively cancelled and you have been absent for two or more semesters, including summer.
  5. You earned a graduate degree from Florida State and wish to pursue a second graduate degree.

Note:

  1. Graduate students who wish to change degree levels (i.e., master's to doctoral or doctoral to master's) before earning the degree should not complete this form. A memorandum from your academic department requesting the change should be sent to the Graduate Section of the Office of Admissions.
  2. Graduate students who wish to change majors before completing the degree should complete a Change of Major card available at the Office of the Registrar.
  3. Graduate students wishing to dual enroll in more than one academic program at the same time should contact The Graduate School to complete a Dual Enrollment Request form.
  4. Graduate readmission will require approval from the academic department with final approval by the Office of Admissions.

INTERNATIONAL STUDENTS:

All International Students seeking readmission must complete a new Certification of Financial Responsibility (CFR) Form in its entirety and submit it along with verification of support. All applicants should mail the CFR and supporting information to: Florida State University, Center for Global Engagement, 110 S. Woodward Ave., PO Box 3064216 Tallahassee, FL 32306-4216, USA.

SOCIAL SECURITY NUMBER:

The Federal Privacy Act of 1974 allows colleges and universities to require the disclosure of Social Security numbers for the purpose of identification and verification of student records, including registration, financial aid, and academic records, and for verification of identity in connection with the provision of its services. The University does not use your Social Security number for student identification. It is used to assign your Florida State University student number (FSUSN).

READMISSION APPLICATION DEADLINES:

Summer - March 1; Fall - July 1; Spring - November 1
